Participating Provider Search

The CSEA EBF panel of participating vision providers now includes both full and partial service locations. Many of the providers still do both the exam and eyewear dispensing. However, some only provide exams and others only dispense glasses and/or contact lenses. When selecting a provider and making your appointment, be sure to verify that the location you are going to provides the service you need. When using a provider outside of New York State, please advise them that your coverage is CSEA EBF administered through Davis Vision. All participating providers must contact Davis Vision to obtain an authorization for services.
