This guide is intended to serve as an introduction to the EBF and covers such topics as a general summary of benefit offerings, how billing documents are formatted, and some of the various forms that a Benefit Administrator might need. EBF ID# Lookup

As a Benefits Administrator you may have the need to lookup members' ID numbers. The following tool is to assist in obtaining the EBF Subscriber ID for your members.
Note: The presence of an ID number is not definitive proof of eligibility for benefits.
